2K Games and Hangar 13 have shown off a new story trailer for their upcoming crime game Mafia: The Old Country at Summer Game Fest. The trailer, called Loyalty is Everything, gives players their first real look at what to expect from this prequel to the popular Mafia series.
The game is set in 1900s Sicily and follows Enzo Favara, a man who survived working in Sicily’s dangerous sulfur mines as a child. Now he has a chance to join Don Torrisi’s crime family and build a better life for himself. But joining means following the family’s strict code of honor, no matter how hard things get.
The story shows how Enzo carries out orders from his Don to help secure power in this violent underworld. Like the earlier Mafia games, “The Old Country” will be a linear story that focuses on one main storyline rather than an open-world format. The developers promise the game will be “gritty, grounded, brutal, and emotional.”

The game takes place during a time when criminals used stiletto knives and sawed-off shotguns called luparas. Crime families would patrol their territories on foot, horseback, or in early motorcars. Deadly family feuds could last for decades. One interesting detail is that the game will feature full voice acting in Sicilian to match the time period and setting, making it feel more authentic to the era.
Mafia: The Old Country will launch on August 8, 2025, for PC through Steam, Xbox Series X|S, and PlayStation 5. The game continues the Mafia series’ tradition of telling crime stories with cinematic visuals and realistic storytelling.
